Have you figured anything out? I am having similar issues. I even go to the code and manually set the widths of columns to inch values, but when I build my target, the column widths are never what I set.

I am having problems with my PDF table of contents. Headings at level 4 appear in a larger font, and larger line height. I have checked the styles for minTOC Proxy, as well as p.MiniTOC1-9.
