Addendum: Do not, whatever you do, delete the first iteration of an article published to SFDC before it is published on SFDC (SalesForce publish action). Madcap support has confirmed that this is a bug. I am noticing a similar issue with other elements nested inside list items but that is a little easier to deal with.

The best workaround I can suggest to anyone on the tables is to put them in a snippet. The snippet will segment out correctly.

Not all classes that exist in the default style sheets appear in the style sheet editor. Inspect the output as suggested here and make sure you are using the same class name in your style sheet so that it overrides the defaults.

Heading level 0 should not be preventing topics from appearing your output, I think you have a something else going on there although I do not know what.

I don't think that hosting something as a web page is the same as dropping the output into a SharePoint file share. If you are opening it by browsing through a sharepoint workspace it is likely just opening the index file and SP could be shutting down execution of the rest of the code, hence no javs...

I am going to assume when you say "print" you mean "included in the HTML output." Try searching for one of the missing topics in your HTML output and see if it shows up even though it is not in the navigation pane. If it does, it is being included but is not appearing in your nav...
