I found a solution to a similar problem. All my topics have a h1 title (maybe a bad idea), but in the PDF, I group these into sections so there is some structure. And I use the "Use TOC depth for heading levels" feature, with Dave Lee's workaround: http://forums.madcapsoftware.com/viewtopi...
