I worked with tech support on this issue and ended up taking the EPS out as a background image on the frame and putting it in the frame contents instead. That method gave me more control over the image sizing.

I still don't understand why the image is not imported at the size that Illustrator ...

I do have a max-width set in my CSS for the img tag. I commented that out but it did not make a difference for the image on the title page.

Changing the frame size does not change the image size.

I am going to submit a bug report for this. Thanks for the help!

The image is smaller than expected in both the page layout and the generated PDF output.

I have been given an EPS file to use as a cover page for a PDF user manual. The EPS file should be 8.75 x 11.25 inches, but when I drop the file into a decoration frame, it is only 6.5 x 8.5 inches (or so).
