Sorry, can't help, only to confirm that I have noticed a problem with MadCap|dropDownHead too.
I've not tried :hover, but I couldn't get :visited to work although MadCap|xref:visited does work as you would expect. I'll report a bug when I get a few minutes.

The pseudo class :hover for DropDownHead doesn't works (MadCap|dropDownHead:hover)...text's color doesn't change on mouse hover.

Yes, you can set the startup topic in the target. The topic doesn't need to be in the TOC.
