I've just had the same issue (never seen it before), and even rebooting didn't make it go away - when I reopened Flare, there was the preview window opened halfway off-screen, with nothing to grab.
Windows Key + arrow key worked for me, thanks Psider if you're still on here after all this time!

Capture does the same - we have three floating licences and at one point I had to call Madcap Support to get them to kill off the extra sessions. Like you, we couldn't see them in Task Manager.

As above - just click into or select the text you want to annotate then the button should become active - but also note that you can't annotate an image (I usually just annotate the text immediately before).
