I assume your output is PDF. If that is the case, you can collapse the top margin with a setting in the PDF target on the Advanced tab.

You haven't specified the scope of the counter. You've given it a name ("H") but no scope. What happens if you specify "GH" (meaning, a global counter, as in, have numbers increment through the entire target) or "CH" (meaning a chapter counter, as in, reset at the start...

What happens if you set either, and be sure to specify which specific page in the page layout file you want to use for that break? Have you looked into the analytics that MadCap Central offers? I haven’t used it myself, so am only pointing you to info about it. The important matter is how Flare calculates the numbers when it builds output, I assume a PDF. In the XML Editor and text editor, auto-numbers will always show their starting numbers in each level counter, e.g., Chapter 1, 1.1, or 1.1.1. To the best of my knowledge, there is not. I assume that you're using auto-numbering for your h1 headings. Flare has been programmed to calculate auto-numbers at build time, so Flare wouldn't "know" what to show in a TOC file, just as it doesn't show calculated auto-numbers in the XML Edit...
