Be able to set sticky nav bar with a checkbox in a skin or target rather than adding 10+ lines of CSS to make it stay put AND get the bookmark links to appear correctly on the screen rather than under the sticky nav bar.

Nope. That is the first thing I tried. It still displays on the online output.
It works fine for me, using that CSS - the text isn't visible when viewed in the browser, but is displayed when I print the topic from the browser.
I can only suggest:

I need to put an indication on printed topics that they are potentially out-of-date if printed. Bonus points if I can include a variable so that I can also have the date and release number automatically update.

Do you have unnecessary files conditioned not to be included? One interesting thing about the HTML5 build is that it will include absolutely EVERYTHING in your project unless it is specifically excluded. Extra files can slow down the build, too.
