The reason there was nothing displayed by the book icon is that, while "experimenting", I had set the TOPIC title to [%=Heading.Level1%]. This is "not supported" (even though it's possible).
I then focussed on setting the book LABEL to [%=Heading.Level1%], for which the html output was [[[Undefined ...

The refresh button doesn't update the display in the TOC editor. So to recap, in the TOC editor, the tree view displays no label if I use variables (system or heading). The grid view displays the variables in the title column.

Not sure if it will help but "For print-based output the TOC labels are based on actual headings within topics."
Is it possible your existing pdf TOC page use a h1 tag?

Using Flare 10.2.1, if my target output file name includes a dot (.), the name of the file created is truncated at the dot.
For example, if my output file is set to "Software v7.0 Service Pack 2" the file built will be called "Software v7"
