I also had this issue in a flimphtml file and it also looks to be related to version control leaving behind some weird information that breaks the xhtml. <<<<<< >>>>> with dates and .mine type messages. I was able to edit the messed up files in a text editor to get them back to valid xml.

What looks to be working for me so far it to set the index options to binary.

Go to the skin > html help setup tab> index options... and check binary.

Has Flare done anything with this? I'm using Flare 2017 and have a single HTML page with multiple headings in it. They are broken for me as well in HTML5 output, but were fine in chm ouput. Of course the whole project I am working on is a series of numbered steps, so that bug with wrapping images and lists is quite annoying. I have gotten a float right for the image to work, as long as the list has paragraphs, and the image is inside of a paragraph tag. here is what the html looks like...

I had success giving the body tag a css property of mc-hyphenate: never.
To do this:
view the css -> View:Advanced
find and select the body tag.
In the Add property window on the right, add mc-hyphenate, and set the value to never.
