For me, the fix was to remove the period (.) from the folder name. HTML5 and PDF didn't care about that, but the CHM didn't build until I removed the period.

The way I got around that error message was to enable XML:
In Word, click File > Options > Trust Center > Trust Center Settings > File Block Settings, then clear the check boxes for "Word 2003 and Plain XML documents."
