Thanks, but it's not the language. After further investigation, I found that of the five buttons in the toolbar, four show my changes to the icon and tooltip but that troublesome PDF one only shows my changes for certain topics. PDF Button in the HTML Output
I was wondering if the PDF button in the HTML output toolbar is some kind of Madcap reserved button. It seems like changes I make to it in the skin have no effect on the output. For example: If I change the tooltip in the skin, the output still displays Show PDF. It does not appear in the right-side...

Thanks. I'll keep looking into it with your suggestions.
As for whether it's always been like this - it's a legacy system so I would say yes. I managed to fix one issue with one button but this mouseover has me stumped so far.
