When you (like me) have variables in H1 and have your cross-refs set to link to H1 then you will know that the variables don't show when you are in edit mode. if you generate PDF I recommend using the page layout files. In there you can easily modify both your headers and footers to your requirements .

I belive this is a codepage issue related to the PDf distiller. I had a problem with Chinese PDF output and submitted a bug report. My issue on the Chinese characters was resolved in V6 of Flare. So I suspect that this could be the issue with the MS Mincho.

There can be many problems (sorry: challengens) in getting table settings right. The problem could also be the printer.
But I would start by checking the html code in the internal editor and see if there are any inline styles that override the .css.
