Problem Solved. I fixed this by adding the following code to the URL:
...//website/filepath/webhelpfolder/default_csh.htm#Content_Folder/Topic_Name.htm|sbs_flare_skin
This applies the CSS to the referenced help file at initial launch.

I got it to work for PDF (a blank page is created after the TOC, with the TOC header and roman numeral page number in the footer), but in Word output, the blank page contains the header of the next book title and decimal page numbering. I can't seem to get a border to appear on images in Word.
I set the "img" style border to 1px border #00000 in the CSS, and the border appears in PDF output, but not in Word.

In PDF output, index appears as a single column, with entries on the left and page numbers on the right.
is there a way to have the index mimic the default setting in Word -- where the entries are separated from the page numbers by a comma and a space?

Are you using a table stylesheet on the table? If so, every time you build the output (and any time you modify the table stylesheet), Flare redraws that table using the table stylesheet, which in effect strips out the td class you applied to the cell because it's not part of the defined table ...

So I am right-clicking the text, selecting Cell Content Style, selecting the style (td.TableStyle.header), and clicking OK.
Is the content in that cell inside a <p> tag or other tag (div, list, etc.)? If so, then you're applying the class to the parent <td> tag but not the tag immediately ...

The PDF output is perfect, but the Word output is messed up: bullets indented improperly, outline numbering in headings different size than text, etc.
