you need to assign page layouts and page types in your TOC (fltoc) file. Right-click the topic in the TOC file and select Properties > Printed Output and select Page Layout and Page Type.

you can add some space before the number using the page number width setting for each pTOC style Properties > Leader > Page Number Width in the simplified view. There's probably a more elegant way to do it...

You can choose to float the text box outside, or to the left or right of the Frame in the Text Box Properties Dialog (right-click the text box div and select TextBox) . I had a quick look and this seems to work for PDFs but not Word Outputs.

What exactly are you printing? Are you printing using the File > Print Option in Flare or are you creating a target (PDF or Word?) and printing that file?

It sounds like a Problem with mediums. You should probably check out the Flare help on this topic. The only things that strike me are the unnecessary margin entries (if you use margin it sets the same value for top, bottom, left, and right) and that you have defined an mc-auto-number-class and mc-heading-Format (=TOC). Can you try commenting out These Parameters and see if makes a difference?

As far as I know the entries in the Table of Contents are controlled by the p.TOC1, p.TOC2 etc. styles. Could you post your h1 and h2 and p.TOC1 and p.TOC2 css code to see if there is anything strange there causing this behavior?

Do your chapters have a different first page from the rest of the right pages? Do you have a 'normal' right page layout? Maybe you could try Setting your chapter break page layouts to Default instead of first right?
