This suggestion got things working for me. I added div.MCBreadcrumbsBox_0 and a.MCDropDownHotSpot_0 to my stylesheet and then and then my custom font starting being used.

Bookmarks seemed to be the issue after all. There were some TOC bookmarks that came over in the Word import. When I deleted them the topics all opened at the top. Thanks for the help!
