Absolutely no idea how or why it fixed the problem, but I recreated the heading in the topic, and dragged it to the TOC and it's fine now...

Thanks for the suggestions. So far, I've only seen partial success. Some tables look better, but the problem tables, those with merged cells - across rows, rather than columns are still misbehaving. no, I have not removed them from the target (I'm not sure I'd know how to do that but I've just checked and I don't think I've done it inadvertently)...

Yes, the variables are in the heading of the topics, and then I have them in the labels for the TOC entry...

I am trying to use manual variable links in my TOC:
[%=Variables_Global.variable1%] and xxx
[%=Variables_Global.variable1%], [%=Variables_Global.variable2%] and xxx
to produce an output of:
abc and xxx
abc, 123 and xxx
