Try setting the mc-heading-level property on your custom style. e.g. set it to 1 if you want it to be a top level heading. I think that will make it appear in the TOC.

You could try unticking the "Include topic when full-text search database is generated" checkbox to see if that speeds up the generation.

I thought I'd add to this. Using TFS GIT, some projects worked fine and others would be extremely slow to check the status, and take 20+ minutes to commit a single change.

Is it SideNav? I don't think there's any way in the skin editor, but you could potentially use CSS to insert text before the actual TOC code.

The missing bit is that in "Replace/Action" section you need to select "remove inline formatting" from the dropdown and re-specify the inline formatting you want to replace.

I think that will style the p following the div, not the p inside the div.
What you could do is set up each div as a snippet, then after you insert the snippet, right-click on it and select Convert to text

I did a quick test and the All Files text displays when I preview, but not when I view the generated output. Check if that is the case for you.

In case it didn't come through, the thought experiment for inline images should have spaces between the apostrophes (so there are correct spaces around the inserted attribute value.

Perhaps you could you ::before and the content property to display the alt text. The content property is interesting because you can use it to display attribute values as well as text strings.

If your organisation has source control, see if you can use that. That way every time you commit/check-in, your work is saved centrally so it doesn't matter where the projects are located. The reason I only recommend using source control if your organisation already has it, is so you can have ...

What's the exact message they're getting? That might help us determine what's happening.
I do know one common problem is if you link to a pdf on a network server and the user doesn't have access to that server or to the specific folder.

Assuming you're using side nav or top nav, you'll need to add a topic toolbar, probably to the template page so that it appears in all topics. If you don't want it in all topics, you can add the topic toolbar to individual topics as needed.

MadCap support have confirmed that this is not a bug and that concept tags must be inserted inside regular html elements inside the body element. That is, inside p, h1-h9, li, div etc tags.

Perhaps check your stylesheet for a div class containing the word "breadcrumb" that has a large padding-top or margin-top set for the print medium?
