Sounds pretty complex.
For what its worth: You could add 2 h1's in the topics, and use conditions to show the correct one for a specific pdf. I guess this will only work when each pdf is an own target in the project.

See last option in auto-number format where H is used as a label. Using K, L M, as a label will start a new global series if formatted as GK:, GL:, GM: etc.,

This is a workspace issue:
open a topic, click the title and make it float.
Then drop it in the correct green block you will see pop-up so it ends up in de correct pane.
Then save the workspace (as default) or with a new name (then you can switch between different workspaces)

to avoid these kind of issues I use the next workflow:
1. always create new styles in medium Default.
2. When really needed another medium, overrule that tag in that medium.

Update (as promised): It has been confirmed that the resulting output in Flare 9 was the result of a bug. This bug has been fixed by MadCap Flare.
As a result of this fix the output in Flare 10, 11 and 12 now is different than in Flare 9.

All I did to get this working perfect is just one step:
In the stylesheet used, find <tr> and set "page-break-inside" to "avoid".
All the other actions you described I did not, so undo them.
Note that the table-styles, if used, will inherit this option, so there is no need to set this also ...

OK, forget I asked this question. It is working as intended, but I had to restart Flare once to get it working.
So when excluded an image stored in the content view as part of a page layout this image is not used in the page layout for that target.
