In the TOC, select the first topic (not the frontpage) and open its properties and set the page layout and select that layout created for topic pages.

I have done some tests using a clean new project. The generated files with and without the stylesheet declaration give the same results. Validation with W3C gives no errors.

What attribute or property in the stylesheet must be used to right-align the autonumbering for the heading levels?
The distance between the chapter numbers and the title must be the same for all heading levels used (h1,h2,h3)

You are right and that is exactly the reason why the PRE-tag is used.
The PRE-tag in Flare 10 and Flare 11 causes the XML to break when the lines in the xml are spanning multiple lines in the documentation (pdf), not only for white-spaces but also for each other character not within [A..Z,a..z ...

Did some tests in MadCap Flare 11 and indeed the bug is still present.
Found Workaround: it is possible to replace each white-space by a non-breaking space and each hyphen by a non-breaking hyphen using the Insert Character... menu.
This will generate the correct output in PDF.
