Hanne, is this for an online output or printed output? Breadcrumbs are not supported in printed output, they are a feature for online output only.
For online output, you can put the breadcrumbs proxy on the master page to make it appear in all your topics.

If your topic displays correctly in the output, I'd say don't worry about trying to fix the preview. The preview can't always display everything exactly wysiwyg, and it has no knock-on effect on the output. If your output displays correctly, you've configured your topic correctly.

You can do this by creating a span class and configuring it to display a border. You can then highlight the desired words in a sentence and apply your span.
