No snippets. The second topic doesn't have any index terms in it. I checked in the Text Editor view as well as the XML Editor view.

No. I put the one-word paragraph in two different topics. I indexed it in only one of the topics. In the generated index, I'm getting two page numbers for it -- one for the occurrence in the first topic and one for the occurrence in the second topic. These are separated by several pages.

I'm using Flare 9. In my PDF output, I'm getting automatic index entries for locations where I have not inserted an index term. I have not enabled auto-indexing. There is no NewAutoIndexSet.flaix file that I can find.
