I suggest to modify your font-size and use em as unit:
font-size: 2.0em
This should work better than pt, especially for responsive themes (see
https://www.w3.org/Style/Examples/007/units.en.html ).

with merge you will get multiple chms as output.
If you want to include all contents in one chm you may need to import all contents into one project. Flare has something that is called global project linking which may help you to organize your projects.

sorry, but I cannot confirm your observation in general: One of my Flare 2017 r2 projects has a pdf in TOC, and when I create a chm, this shows the contents of the pdf as expected on my Win 10 Version 1607.

any Flare project can have as many targets of as many different types as you want. This has been exactly my recent suggestions.

seems to be some kind of version conflict. My Tripane HTML 5 result created with current Flare and with left contents pane works well, with FireFox 54.0, obviously ecxactly as with IE11.
If you have another FireFox version, please let me know. I will test it, if possible.

For changing location of toolbar icons you may find Setup in your skin which offers left or right positioning. Changing the sequence of icons is done in toolbar section.

Renaming chm file isn't good. You'd better change name of generated file by using the property base name in D2H's Help Target dialog. I am sure that this will keep your links alive.
