This looks like you are mapping images as External Resources, which you don't need to do unless they are collaborative files stored on some network mapped drive or similar. Just drag the image into your actual Flare Resources/Images folder and it should never go anywhere, unless you use source ...

The only time this has sometimes happened to me is if I've forgotten to put a class on the image or resize it and it has a large native size. Sometimes it'll be in my output cropped but sometimes it doesn't appear at all, this isn't 100% replicable however.

Yes this is possible, you will have to assign an ID to your icon and use some jQuery.

This is definitely an out of scope question for these forums, but MadCap Central does allow SSO authentication, the only problem is it only allows one IDP to be connected, which typically would be your organisation.

A cursory google search suggests this error may be due to an invalid space in an xml tag i.e. <tag > instead of <tag>, but this is just best guess.

This might well be related to the "Auto-end on 'left' page" setting in the TOC (right click on the topic, Printed Output).

It's a little bit more than you asked for, but I dug around and this stackoverflow answer offers a solution, albeit it utilises a grid display for the list and you need to change the alignment from right to left:

This looks to me like it's the double digits forcing the icon to be pushed to the left. Can you increase the right margin for the list?

Do you have Git for Windows installed? There seems to be a variety of problems with Flare's own Git implementation. Ah apologies, I thought perhaps that you were a bit further ahead in editing PageLayouts.
Add a Decoration Frame to your PageLayout, and click it, press F2 and click No.
You'll then see a window open up to edit the Frame contents with a paragraph block in by default.

You can do this with variables, just note that you won't see any images rendered at all in the PageLayout until you build the output.
Just use something like this:
<img src="../path/to/barcodes/[%=Barcodes.BarcodeType%]">

If you drag the TOC over your "main" Flare window, you should see some layout buttons appear.

Do you use source control? If so you could create a User condition tag set and add it to your .gitignore so each author can maintain their own version of the set, which could then be used in meta tags.

Crikey. Add this to the list of incredibly frustrating areas where Flare is several years behind the industry standards. I really do worry that the SaSS offerings are getting so much better and Flare seems to be frozen in time...
