I got a reply from support and wanted to share it in case others have the same problem.
The mc-auto-number-format property that was set for the h2, h3, and h4 elements in my project were set to

I tried to change {n+} to {n} this but that does not fix it. Instead of all the numbers being 3.1 and 3.0.1, they all turn into 3.0 and 3.0.0, but they are still repeated and do not increment.

Can anyone help me out here? I have a manual written in English and my colleagues used MadCap Lingo to localize it to Arabic. Now the numbering has gone wrong.

I have found the solution. I asked for support from MadCap and they answered that Flare 2018 does not support FrameMaker 2019. In order to build a FrameMaker target, I need to use an older version. This would have been nice to know before investing in the wrong license and spending time installing ...

Perhaps I should add that I have tried the suggestion from Flare's help to run Flare as administrator, still no effect.
Update: I have now a licensed version, and I can still not generate a Framemaker file. Does this work for anyone else?

So this means that if you have a topic you are working on for later use and do not want it included in any searches, you need to condition it out in the Content Explorer and not just in the TOC? Wow, I did not realize this. Game changer for me!
