Right click the image and choose one of the Text Wrap options "In Line with text", "Square", "Behind" or "In Front of". Once that's done you can right click the image and in Properties set any margin/padding you want for the image on all sides or just one or two.

Right click the entry in the fltoc file and choose properties.
On the printed output tab change the Break Type to Chapter and set the appropriate page layout.
In the Page Number section, resest it to "A" with the format ALPHA. That should do the trick.

Yes you can. In the Page Layout Editor, right click the frame and choose Rotate. The contents will rotate with the frame. This is true for Decoration, Header, Footer and normal Body frames.

I suspect the style has been added to the print medium and not the HTML5 one. Verify that when you are changing styles you change them for all appropriate mediums.
