If this is something you do regularly, could you encase the CD section in a DIV then use a background image on the DIV to include the CD image?
For example, I have a Caution DIV style that puts a standard Caution icon in an indented margin:

Sounds like syles aren't being found and default fonts are being used.

Not sure I understand what you're trying to achieve. It sounds like you want the H1 on the second page to be the same as the Title. How about using two variables? One variable for the URL and one for the text. So if a target has the URL defined it gets turned into an appropriate link. If there is no URL defined for the target, just the variable with the text in it is displayed:

Check also the topic isn't conditioned in the TOC, and that the file itself isn't conditioned in the Content Explorer.
I'd look at the .fltoc file and the topic file in a text editor just to confirm there are no sneaky conditions being applied.

The caveats: each page layout needs to have exactly the same page types, and no more than one of each - because when you don't select a page layout, you just get the basic list (e.g. normal, title etc).
And you need to be prepared to have the page layout breaks fall at the same topics for each ...

If you can define a single page layout for each output, you can define the page layouts in the targets and avoid specifying them in the TOC entirely.

That said, MadCap Flare is apparently embedding the font incorrectly. If the font were embedded correctly by Flare's PDF engine, this wouldn't be an issue. So I hope MadCap is also looking at this for a fix from their side, so if Adobe decides again at some point to strictly enforce font encoding ...
