I do love a challenge... The closest I could get is to use the ::before pseudo element to add in a line break before the "See also" span. Being a line break, it normally has the same indent as the element containing it, in this case the p.index1 tag. So you have to fudge some whitespace in there too ...

Have you tried using the code snippet tool to highlight code? Insert -> Code Snippet and select the appropriate language. SQL is one of those. The drag and drop in Flare works well I've found, fixing links as it goes. The worst that would happen is when a target gets built you'd get an error stating the page layout was missing. If you get that, update the target to point to the right one and you're golden.

Just wondering if you are working on a network drive? I used to find slow performance whenever Flare had to build because of the network speed and the framework Flare uses. Without seeing what's in your code and css it can be hard to diagnose.
As an alternative (and CSS/Flare is usually full of alternatives), could you change your "example" class to be generic and then be able to apply it either to a paragraph singly or the div as a group? So:

Can you link your cross reference to a bookmark on a page instead of a heading? That way the bookmark will be collected by the cross reference and no extra TOC headings. I do that when I want a link to point to something further down a page than the very top.

Don't know if this is the 'right' way to do it, but I mark all my UI terms in a span with the class "UIElement". Then advise the TX team to not translate text in those spans. In my CSS they are defined as bold text (making it easy if I decide to change from bold to a colour or something else).
