If you want to do linking between the books to concepts in other books, then I'd recommend a single project so you can have cross references, where at least the title will stay up to date if it changes in the source topic.

I imagine that could also happen if the page is conditioned out in the Project Organizer, but not in the TOC, so you wouldn't even see in the TOC that the item had been conditioned out.

I think A is most likely your answer in this case. I think providing Word and PDF items is not a common (not unique, just uncommon) use case, so people haven't seen it yet.
