do you have the appropriate style medium set in your Word target properties? are the correct styles showing up in the Word doc, but you just don't see them in the build process log?

sometimes the preview is also different from the output, so be sure to check the output too. yes, it is annoying that sometimes the editor, preview, and output don't match, but it's really just the output that matters.

where are you seeing this dramatic difference? does it show up just in the flare editor, or only in the output? what kind of output?
have you made any adjustments at all to the ol, ul, or li style properties? does this happen in a sample/new project?

The TOC title only uses the H1 setting when you originally create the TOC item. It will not update again automatically. it is just the way Flare is. you can submit a feature request here - https://www.madcapsoftware.com/bugs/submit.aspx

if you use a percentage-based font size in your CSS, your users should be able to adjust the size of their fonts in their browser. (ctrl, + or - in firefox)

do you have it set for your heading levels to inherit the toc depth level? it would change the h1 to h2 since they are subtopics.
if you want to sometimes have h1 with a page break and sometimes without, you can add a class to your h1 style ("h1.break" or "h1.nobreak") and change the page-break ...

you could just make sure your toc names are short enough that the user won't have to scroll to see the titles. to me, it seems just as easy to scroll back/left when the screen shifts as it is to scroll right to see the whole name.

I think the only way to do this is to make the top level "book" not linked and move the topic with the parent-level content inside the book.

When I evaluated X-Edit, I determined that it was too complicated for my SMEs to learn just to review a document. I usually send them a Word version to mark up with changes highlighted/in a different color/tracked. I haven't tried Contributor, but it looks like it would be the same type of interface.

do you have the TOC item properties set correctly for those sections? make sure you pick the appropriate page layout (two column) and set the "end of left page" to disabled, and make sure the following section has continued numbering.

do you have any extra spaces/styles at the end of the 2 page ...

are you talking about the margins/padding for the header frame? it could be that your logo/image is pushing it over or there is white space around your logo that makes it look like the margin is different.

what difference are seeing? the top padding and the right margin of the heading style? that may just be differences in browsers.
And yes, a network project viewed in IE without enabling MOTW may have some issues (with the security pop-ups) that don't show up in FF.
