Usually if something doesn't show up in the header or footer it's because the content is too tall for that header or footer. You generally don't want to do that in non-Flare websites either, unless you have your web server set to allow directory views. That's generally not recommended from a security perspective, I don't think, unless it's an FTP site and you have it walled off from other websites/servers.

... and the reason is because online outputs automatically include all content in a project, unless the content is excluded using condition tags or that option that Tom pointed to. Print outputs, on the other hand, only print topics that are listed in the TOC.

Look at the stylesheet to see how you have the <ol> tag defined, and if you have an ol li complex selector, look at its definition, too. You might have set line-height to 0 or set some other attribute that's causing the effect.

I'm not sure what you mean by the Project Properties window.
To have the TOC appear in the printed output, you need to create a topic and insert the tocProxy in that topic, then add that topic to the TOC file (via the TOC Editor) that your print output is using.
