Maybe you should google the error text. That might help you in getting an idea what is causing the problem - might not be Flare or VSS but sth in the operating system.
Oh, and: We are working with MS VSS 2005 since 2008, without a lot of problems. Most of our problems come from Source Control ...

Sounds like it's a problem with the DOM (Document Object Model). Each browser has a different one and it even differs from one browser version to the next. - Welcome to the wonderful world of web development! Would be a good job for Sisyphus.

OK, your first suggestion worked great for online output, but as you mentioned, the image does not show up in printed outputs.
Background images are always difficult for print. And even if it might theoretically be possible, the user might switch it off.

This means you also have to educate your tech support staff about the help, and include references to the help in knowledgebase articles, so support see those references when they look in the KB, because support might very well not read the help, or might only read it when they first start with the ...

Are you working with Source Control? Then it could be that somebody else has checked out that file and it's blocked for you. Or that you have more than one version of the project on your local computer and you have it checked out in that other version, but not in the one you're working in.

The "print" medium of that stylesheet is used for printing topics from the help and the preview. You might have to add those declarations to that medium, too.

You could apply the margin-left (or padding-left?) to the BODY and only assign a negative margin (yes, this is really "margin" and not "padding"!) to those elements that you want to be further left - depends on what sort of setting the majority of your content has.

A topic isn't aware of the calling context and how often and in what paths it appears in the TOC - it simply doesn't have that info.
And, since you're not working with a snippet, that topic exists only once. And the place in the TOC it jumps to is its first appearance in the TOC ...
