The technical solution here looks good, but, in my opinion, one overlooked issue is that the graphics dominate the text and may impede usability.

I ran into a similar challenge when importing an MSWord doc with multiple tables. All table cells were formatted with <p> bound to <td>. Here's the way I was able to unbind all the content in a table's cells at once:
1) Select the table rows and columns.

Flare seems to create a td class <td class="TableStyle_TableOnline_Body_0_0_RowSep_ColSep"> that doesn't show up in the topic CSS. The settings are probably in there. Do I add this class to the topic CSS by using the text editor and just set a font size setting?

These snippets don't supply all of the topic's content, and they are used in various contexts. Headings are a challenge in reusable content: As the context changes, hierarchy becomes an issue.

In the PDF Target Editor, on the Printed Output tab, do you have "Use TOC depth for heading levels" enabled or disabled? If you have it enabled, then it will set up the TOC with the lower topics indented under the parent books. So if a topic is three levels deep (indented) in the TOC, then it will ...

td and th font sizes in print medium
My td and th font size settings (print medium) aren't working properly in PDF output. Both p tags and td have print medium settings of 9pt, but td results are visibly larger. Any thoughts? The WebHelp output looks exactly as it should (font settings for p at 1em and td at .9em).

Yes, I much, much prefer going directly to PDF vs. going to FrameMaker and then PDF. All the post-build work in FrameMaker was throwaway work. I've now eliminated almost all the throwaway work.... It took awhile to figure it out, but it's now working.

Another way to resolve this is to move the "Index" title flush left. That will mask the effect you mention.

Flare v. 4.2.1--I have exactly the same issue. I tried the same seemingly approach and saw no change in the PDF output.

"Use TOC Depth" is enabled. I think it's set up right. I think garyjgeoaccess has it right... probably set h3 to 0, but I don't see that control in the h styles or TOC styles. I find mc-heading-format, but not "level."

TOC Depth in PDF output
Somehow I've managed to restrict TOC depth to two levels. How do eliminate the restriction? I can't find this setting in the CSS or in the TOC properties. I have topics in the TOC that don't show in the TOC or the book.

...tables in PDF output are either 4.5 inches wide or 6 inches wide, depending on the table style selected. If you need to tweak column width, use percentages (and don't expect that Flare will really pay much attention to what you enter).

Any news on this for PDF output (Flare 4.2)? I need to get the chapnum-pagenum format for my generated TOC. I would like to set this up to remove the chapnum that now displays before the heading and instead get the chapnum-pagenum construction flush right.

I think you need to preface your mc-auto-number-format style with GH, not just H. This turns on a global series. And keeps the numbering going from topic to topic. Otherwise, the numbering keeps getting reset from topic to topic.

I adapted my FrameMaker autonumbering styles, which hold numbering locations for figures and tables in the final two spots. Here's the scheme:
