If you take a look in the Output>Temporary folder, you should be able to find a folder with the name of your PDF target, with htm files in it. Yes (separate topic with glossaryproxy, separate TOC containing that single topic, PDF target referencing that TOC). I can't think of any other way of doing it

Good result! Pleased that you found the problem. We use Calibri and that comes through OK (I find that font gives the best result at smaller sizes in callouts).

Flare uses distiller to create JPGs out of anchored frames with images and callouts. Maybe there is something wrong with your Acrobat installation - issues I've had in the past were resolved by uninstalling Acrobat (or the PDF tool that comes with FrameMaker), and reinstalling. It does not include IE 10, which is currently included only in the dysfunctional Windows 8 Consumer Preview.
