I should note that I only have IE 8 installed and could not test IE 9.
One other note, I tested this initially with the HTML5 output, but I just tested this with older webhelp output and the result was the same.

Lisa is correct. DITA supports a draft-comments element. Since we're partly a DITA shop, I have to support that tag in my css. What I do is publish the draft comments in our review/draft stage using conditions, and turn off the comments when I publish a final copy.

Setup a global number type and it will do exactly what you want. The Prefix format is:
Gx:
where x is any letter you want. I use L for lists P for paragraphs, etc.
This is covered pretty well in the help by the way. You may want to look there for additional formatting.

In tables, you must use caption in order to get the continued tag in your table captions. It sounds like you may a paragraph style you use in place of the caption. If that is the case, then you will not see (Continued) in the text.

I have the exact same issue. I worked around it by using the old HTML trick of making the top of the graphic "taller" with white space set to invisible. I never found a better solution.

FYI, I tried to get the .NET help to work with Mono on Lion (Mac OS X 10.7) and it didn't work. I get an error that states: File Does Not Contain A Valid CIL Image.
