Check that the @print medium in your stylesheet is modified, not just the @web medium. What does it look like if you generate a web output - is it the correct font there? You might need to look at the stylesheet in text editor, just to be sure.

There is a style called Madcap:conditionalText, so you could start your investigations there. If you add a background colour (or something!) to that style and then regenerate, that might help. I haven't done it myself but it's probably worth a go, and would be easy to put back again if that doesn't ...

This might be a completely silly suggestion (feel free to ignore it, or the lovely Lisa will tell me I've eaten too much stupid-cake today) but could you put the caption in another Header row? If you did that I am just guessing that the caption should repeat as well as the table heading.
