I've seen this happen. Or, some variation of it.
I switch to text view and edit the word there. In most cases, I find that after making one edit, I can switch back to the XML editor and the problem has gone away, letting me edit normally again.

Not exactly answering your question, but what if you selected "User lowercase filenames" in the advanced tab of your target?

In the meantime, you can always open the text editor view and move the tags to put the table inside the drop-down body.

If you're generating print, that should happen automatically. Otherwise, not so much.
There is a method you could use, but you might not like it. Replace every heading and matching TOC entry with a variable. Both will update from the same definition, and therefore they'll match.

I would add a variable. In fact I do exactly that, but I use the date rather than a version number, and I tuck it away in a corner of the welcome page rather than the header.

Tricky. The Word output is the easy part - just create a new target. The hard part is the formatting. You need a way to take a tag that looks like this:

So far as I can see, the answers are no and no as you've already guessed. But, there are two tools that are kind-of, sort-of close to what you want.
1) You can turn on the grid and set the spacing.

I've been experimenting with my own Flare-generated .chm file, hoping that I would be able to contribute some ideas, but I can't make it not work.
When I run, HH -mapid 10200 htmlhelp.chm (or whatever number) I get my topic every time.

Following up... No bugs here. Flare remembers the last folder that you used when inserting images. For some reason, after I upgraded to 12, it reset the path to the project folder rather than the \resources\images sub-folder.

Without seeing the css, this is just a wild guess, but what do you have the opacity of the content area set to? I'm guessing that it's semi-transparent.

Not really. I see DITA as a good way of organizing content, especially for larger organizations. But, from what I've been able to find, the tools available for presenting that well-organized information don't do nearly as good a job as Flare.
