You would do that with a button that has an onclick event, same as for the tabs themselves. You're working with javascript to change what's displayed, rather than html to jump to another page or target within a page.

I believe that for PDF outputs, you need to define the columns in a frame of your page layout.
At least, that's what it says in the help file: Features > Elements and Objects > Page Layouts > Columns

It also depends on how you want to go about it. With some outputs, you could use the Madcap embedded viewer and call that. Or you could generate a url that includes the CSH ID value. Or, yeah, there are options ...

Interesting question. According to the list of options in the spell checker, you can check topics, snippets, TOCs, glossaries and index files, but not variable sets. Seems like an oversight. I suggest submitting a feature request.

You should be able to edit your TOCFileName.fltoc. (Make a backup first!)
Just add BreakType="chapter" to every TocEntry that doesn't already have one. If you have a text editor that supports macros, it should go pretty quickly. Otherwise, it's just a few minutes of copy/paste.
