Random page numbers in TOC

In the pdf TOC there are just 2 random page numbers with no content referenced. Something like:
........................... ....... 7
Updates.......................... 7
Topics............................. 64
.................................... 64
Topic Discussion ................ 68

The headings are all formatted the same in the TOC grid view. Help?
