Making Changes to top of Help Topic

Good morning,

I am in the beginning phase of creating my newest online help system. I've figured out how I want a lot of things, but I'm having difficulting making some changes (space reduction and adding rulers - to be exact). This is not in reference to the Home page, but to regular topics.

I am using a toolbar proxy, Top Nav skin, with OtherTopicsFixedSideMenu master page and skin with HTML5 - Top Navigation FixedSideMenu as a target.

I have tried tweaking each to make the adjustments i need and I don't seem to have been successful thus far. I believe part of it is that i'm not used to working with Divs...

I'm attaching an image showing what it looks like now, the changes I want to make, then a mock-up of how I want it to look.

Any advice or input would be greatly appreciated!
