Flare 12 submit errors to Perforce

We have been challenged with intermittent errors when submitting Flare changes into Perforce. The "Submit All" feature seems to succeed in Flare, but when we get the latest revision from Perforce, sometimes images, TOCs, and Topics are missing. Does anyone have any insight to why this is happening and how we can avoid these errors?
