Printing the master TOC and associated filenames

Does anyone know of a way to obtain and editable form of the master TOC? What I need is to have a text document that contains the chapter names as they appear in the TOC and the corresponding HTM filenames.

Right-click the TOC and select Open With > Internal Text Editor. That should give you the XML version of the TOC, and it's relatively easy to pick out the text you want from there.
