Hi All,

We have a requirement for a manual to create revision history on top of every page. It should show when the page is changed and what is the issue number.
Record of Revisions
Section : 03A [Fixed]
Page No. : 01 [Fixed]
Issue No. : 04 [Should increment everytime there is a change in the page]
Date of Issue : 09 Dec. 2010 [Should update the current date on which the change is made]

Point 2:

In the beginning of the manual, there should be a summary of list of changes done.
