I recently began converting some older content to newer HTML5 templates. Most of the content was looking pretty good, but I noted that a couple of pages were not showing the top or side menus. The settings for those topics and their TOC entries matched the settings for topics that did show the top and side menus, so that pretty much ruled out style settings. Then I realized that the two topics had a similarity in their H1 headings: Both used single quotes. The 'Open' Dialog and The 'Save' Dialog.
Changing the labels in the TOC made no difference, but editing the topics -- removing the single quotes and either italicizing, or using double quotes -- fixed the 'missing menus' issues in the HTML5 output. Of course, the menus don't show the italicized content, but the topics do. The double quotes look too busy in those short topic titles, so I'll probably stick with the italics.

Could it be that they used curly quotes rather than straight quotes? Curly quotes can sometimes mess things up.

A good thought, but no; these were straight single quotes. I also tested adding and removing the single quotes from within Flare, and recompiling each time. Just now, I put single quotes around an h1 header that had never had any punctuation and recompiled. Sure enough, the top and side menus for that header didn't populate. (I should have mentioned earlier that the breadcrumb proxy also disappears.)
