Creating a Contact Form in Flare

Is it possible to include a Contact Us form within the HTML5 output? I've searched around this forum and the only post that mentions a form is this one: viewtopic.php?f=6&t=28124&p=123539&hili ... rm#p123539. Unfortunately it's from 2 years ago, and only refers to a syntax error in someone's code.
