ToC entries not appearing in webhelp plus output

As I recall, some modifications I had made to the skin resulted in an error or blank nav pane and toolbar when I viewed output in IE6, but it was fine in Firefox. I didn't have IE7 at the time, so I don't know if the problem existed there as well.

To test if it's something in the skin, create a new skin, don't make any modifications to it, set that skin to be used by the WebHelp target, then build the output and see if the nav pane is still missing.
