Adding Index to Print Table of Contents in PDF

I've been testing different solutions to add the Index to the printed table of contents. I've added

1; to different classes in the stylesheet and that hasn't worked. I've added a

system variable to the Page Layout header and used the Heading variables for the Indexes. So far no success.

Is your index generated in the Target properties or have you created a topic with a index proxy in it?
