I am getting this error for every page in my document.

Are you using a masterpage? If so, what do you have in it (breadcrumbProxy, bodyProxy, etc.)?

I've never gotten this error with every topic, but I've seen it come up in the following situations.

1. The topic IS in the TOC more than once. In my Help, for example, we have a book section for employees and one for managers. There are some topics I include in both books because both sets of users need the same information, but in different contexts. So in this case, I get the error message, but nothing is really wrong--it's working just the way I want it to.

2. When I first started using Flare (after being a RoboHelp user for years), I would link the Overview topic to a book, then include the same topic IN the book. This was a true error and the error message helped me get clued in. :)

Could either of these situations apply to any of your topics?

Tip for your scenario 1: instead of adding the topic twice, make a snippet out of everything except the top heading. You can then make two different topics (so they will be unique in the TOC and in breadcrumb trails etc.) but not have to maintain two different copies of the content.
