My apologies if the subject line is a bit redundant -- or obnoxious -- but that's what Flare's doing. It's inserting the following tag after every single frappin' <p tag:
xmlns="http://www.w3.org/1999/xhtml"
It's even inserting this tag after some <b tags and some <div tags.
To clarify, I've left off the closing carats here in this post on purpose, to show where the insertion is taking place. What I'm seeing is this:
<div class="note" xmlns=""><b xmlns="http://www.w3.org/1999/xhtml">
or
<p xmlns="http://www.w3.org/1999/xhtml">
What does this mean, and how can I turn it off?

I've just done a quick google on xmlns and it came up with namespaces, which are used when an attribute is used in several different circumstances. (I note, as you probably did, that if you search the Flare help for xmlns or namespace, you get a big fat zilch as a result.)
Take a quick look at http://www.w3schools.com/XML/xml_namespaces.asp to see if that will explain WHY you're getting this, as it looks like there's some kind of conflict with the names of your attributes. Without any further information about your paragraphs I don't think anyone will be able to help much, but it could be a useful starting point for your own investigations. Good luck.

There should only be one xmlns declaration and that's at the top of every topic. I have seen a situation where the xmlns declaration was in some tags in the content, but I think that was an imported topic, so I figured it threw them in when it was converting the topic to XHTML (i.e., the thing it does the first time you open an imported topic in Flare). Try creating a new topic from scratch and inserting some paragraphs and see if the behavior continues.

I am seeing this behavior in some projects and not in others.
I will see if importing content is the common element but I don't think so. If anyone has insight on how to stop Flare from adding empty xmlns attributes please post here but I keep removing them and they keep coming back.

The year is now 2025 and I ran into this issue. It's been happening for a few weeks now, but not consistently. It seems xmlns="http://www.w3.org/1999/xhtml" is added to <span> and <img> elements. Is there a solution?

I haven't seen this problem with the same xmlns reference, but I have seen it for xmlns:MadCap="http://www.madcapsoftware.com/Schemas/MadCap.xsd".
Topics should include this xlmns reference in the html tag:
But for some reason, Flare sometimes removes that from the <html> tag, and when it does it will add an xmlns reference to every MadCap: tag in the topic.
To fix this, I copy/paste the xmlns reference back into the html tag, and when I save Flare reformats the topic and removes the additional xlmns references on the MadCap: tags.
So I would check the topic html tag, and insert the standard MadCap xmlns reference if it's missing.
