Previous Next buttons inconsistent

We have built a WebHelpPlus output which includes Previous/Next buttons based on the TOC (no browse sequence). When we access the default.htm directly from the www\root\webfolder in a browser the previous/next buttons operate consistently. When we access it using Localhost or the machine IP there is a section of the TOC that jumps to incorrect topics. I have spent hours trying to figure out what the problem is with not success. The buttons also operate correctly when we produce CHM output. Can anyone offer any suggestions?

Did you delete the WebHelp output folder before building? It could be that you're looking at a cache of the output or a partial cache. Delete the WebHelp output folder then rebuild and see if that fixes the problem.
